New space enhances access to personalized, whole-person care for seniors
TrueCare, a nonprofit community health center serving North San Diego and Riverside Counties, is excited to announce the expansion of its Older Adult Health Center in San Marcos, strengthening access to comprehensive and personalized health care for our aging population.
The expanded health center has grown from 1,800 square feet to more than 4,300 square feet, allowing TrueCare to significantly enhance services and capacity for seniors living with complex health issues. The expansion includes four additional exam rooms for a total of nine exam rooms, a new community room, and a larger lobby designed to better accommodate seniors and their caregivers.
TrueCare’s Older Adult Health Center is specifically designed to address the unique and individual needs of the senior community. This specialty clinic offers highly experienced internal medicine physicians who are passionate about delivering personalized care for seniors living with complex and chronic health conditions. The senior-friendly environment supports longer appointment times and a team-based approach focused on whole-person care.
Patients have access to primary care along with chiropractic services, podiatry, behavioral health, virtual health, care management, medication management with an onsite pharmacist, and additional supportive services. The expansion also introduces dietitian services and expanded behavioral health care, with the center anticipating nearly 17,000 patient visits annually. By offering multiple services in one location, TrueCare makes coordinated, high-quality care easier to access.
